Please complete the forms below and bring them to the school along with a copy of your child’s birth certificate and proof of residency at the time of registration. The school requests a physical within 6 months of beginning school. Please have your child's health care provider complete the Universal Child Health Form and his/her immunizations at their 5/6 year old check up These forms must be submitted to the nurse prior to the start of school.
